Our Hospital In The Home (HITH) and acute programs aim to provide flexible, person-centred care for patients within their homes.
Pop-Up Health provides highly skilled acute care nurses to assist in a smooth transition from hospital to complete treatment at home, reducing time spent as an inpatient or avoiding a hospital admission all together. A specialist nurse will visit when required to assist with the onboarding and complete the referral process.
We service both public and private hospital systems through various partnerships and agreements.
